Pre-Implementation: The DNP student will post flyers in her community church in Texas regarding the potential for participation in the project, and to introduce the details of the evidence-based intervention to the church congregation. Attendees who would like to participate will be informed on the details of the project, and asked to sign a written informed consent by the DNP student. The DNP student will then obtain the participants’ baseline weight in pounds and ounces and height in inches individually in a private setting, to calculate baseline BMI. The intervention will consist of 8 (45-minute) weekly meetings in the church meeting room immediately after the Sunday service. The DNP student will provide the participant with a journal in which to record their daily diet and exercise activities and their feelings related to weight loss. The participant will be asked to bring the journal to the 8 weekly sessions to be able to share their progress or setbacks, and/or to ask any questions that they may have on a weekly basis. The DNP student will lead the weekly meetings, and will be able to provide formative support throughout the project.
Implementation: The participants will meet in the church conference room after the Sunday service for the educational portion of the intervention. The DNP student will apply the 5A’s behavioral framework and obesity guideline for obesity management. The components of the intervention include:
(a) Asking permission to discuss weight;
(b) being nonjudgmental and exploring the participants readiness for change;
(c) assessing the weekly weight in pounds and ounces;
(d) exploring drivers and complications of excess weight;
(e) advising the participants about the health risks of obesity and the benefits of modest weight loss;
(f) the need for a long-term strategy and treatment options;
g) agreeing on realistic weight-loss expectations, targets, behavioral changes, and specific details of the treatment plan and assisting in identifying and addressing barriers; and,
(h) providing resources, assisting in finding and consulting with appropriate providers, and arranging regular follow-up. The DNP student will conduct formative evaluation through the weekly weight checks and by auditing the participant’s journal. Time for questions and support will provided at the end of each session.

Pre-intervention and post-intervention weight and BMI will be collected/calculated for summative evaluation
Formative evaluation will be use of a compliance checklist to record and determined by attendance at weekly meetings, weekly weight and BMI measurements, and daily use of the journal.
Post-Implementation: The DNP student will obtain the final body weight and BMI for each of the participants. Data analysis on individual pre/post weight loss will be conducted by the DNP student with the support of a statistician. The DNP student will make a plan for sustainability based on interest from the congregation. The participants will be able to keep their journals to continue to use after the intervention. The DNP student will present the results in a final meeting with participants, and a summary of project results will also be shared with faculty and classmates.
DATA Collection: 1 Pre-intervention and post-intervention body weight and BMI will be compared to determine if there is a significant difference based on the evidence-based intervention;
2. Compliance will be determined by attendance at weekly meetings, weekly weight and BMI measurements, and daily use of the journal.
