Observe and track your health behavior and any intervention strategies you use to try and change it during the next few weeks
Please make your best effort to change your health behavior, remembering all of the concepts we’ve discussed in class. Work towards achieving your SMART goal using the intervention strategies or strategies you wrote about in Submission 4. If those aren’t working, feel free to try something else!
Document your health behavior using a numerical value for each day, just like you did for the baseline tracking. You must track the behavior for a minimum of 15 days. Plot your values on a graph so that you can see how your health behavior changed over time.
Feel free to use the same chart or file you created for your baseline tracking, and update the data. You must use a table or spreadsheet of some kind to generate a graph/chart (it cannot be hand-drawn).
Please also document the intervention strategies you use. You can track them daily, alongside your health behavior, or you can keep a separate section of notes and write down what you tried week by week and whether it helped. This part is up to you. You can either include these in your graph or chart, if you tracked them numerically each day, or you can submit a brief summary in written form.
