You recently participated in a local health and wellness fair. One of the booths you visited was offering a chance to win a free wellness assessment and health coaching session. To your surprise, you won! As part of the process, you are assisted in designing, carrying out, and evaluating your experience for changing a personal health behavior.
Instructions
Behavior Change Declaration: You state what you want to do. Address the following:
*The reasons for your choice
*What you hope to learn or achieve and why
*Any prior experiences that are similar
*The ways you will determine progress
Research: You find resources that provide information about your proposed Health Behavior Change. Consult books, magazines, journal articles, or the Internet to accomplish your goal(s). Because research in the health field is extensive, find resources that are no older than five years. For each of the resources, report the following:
*The title
*The author or writer
*The source: name of magazine, producer of video, affiliation of professional expert, Internet address
*Date of publication and pages on which the information appears
Health Behavior Plan: You develop and describe a plan for carrying out your Health Behavior Change. Describe what you plan to do for your Health Behavior Change and the ways you will determine progress.
Assessment: Write an evaluation of your experience by:
*Summarizing your project plan and the health principles it represented
*Listing at least two things about your topic that you learned
*Describing what doing the project helped you learn about yourself
*Describing what you learned about how to change a health behavior
