Submit a 2- to 3-page evaluation of criteria used to select a partner organization to lead a health initiative (excluding title and references pages), to include the following:
• Describe what criteria you followed to select the organization to partner with and why the criteria are important.
• Explain any concerns about who will lead the initiative in your community.
o Will your facility continue to lead the initiative once it is implemented or will the partner organization take over the program?
o Does it matter who oversees the program? Why or why not?
• Assess any benefits of not partnering with an outside organization/agency to address a community health need.
• Analyze how the social determinants of health in your community could impact which potential partners you might choose to work with.
o Could you use them to your advantage or would you need to address the SDOH to work with the partner organization?
