You are required to complete an intervention paper by the end of the semester. A proposal worksheet regarding your topic, the theory you are using, and the intervention plan, is required three weeks before your paper is due. More details will be provided in class.
a. Find/ Research a case-study or a health topic that you want to address.
b. Identify the health issue and the intervention population.
c. Gather information (literature review).
d. Identify possible causes for the health issue.
e. Identify the level of interaction (interpersonal, intrapersonal and/or community)
f. Identify the theory or theories that match best.
g. Submit a detailed description of the intervention.
h. Provide a concept map of the theory used with specific information pertaining to your topic/intervention.
g. Relate to course concepts and class discussions.

Rubric:
Paper length ( 4-5 pages double spaced, times new roman, font size 12): 30 points
Research efforts (at least three articles should be researched and referenced): 40 points
Organization (flow of ideas and following the sequence of the above): 40 points
Concept/ Theory Map: 20 points
APA format(reference page, running head, in-text citations and cover page): 20 points
