Fire in Baton Rouge
CSB Safety Video about the November 22, 2016, fire that severely burned four workers at the ExxonMobil refinery in Baton Rouge, Louisiana.
After viewing the video linked above, conduct an evaluation of this incident, and include the following information in your paper:
Discuss process safety management (PSM) in relation to the incident, including regulatory requirements that pertain to this type of incident.
Explain briefly how each of the 14 PSM regulatory requirements would relate to this type of incident.
Analyze process hazards that existed prior to the commencement of the work. What methods would be used in the analysis?
Summarize the emergency response plan requirements that would apply to incidents of this nature.
Discuss a minimum of two control measures that might have kept this incident from occurring.
Discuss one or more other hazards that are commonly found in the process industry that might lead to or contribute to this type of incident.
Identify at least two root causes that led to this incident and, had they been abated, would have kept the incident from occurring.
