Human Diversity Project (Professor’s Instructions)
Purpose: The purpose of the Human Diversity Project is to apply human behavior theories and the Social Determinants of Health (SDOH) framework to understanding the challenges faced by an at-risk population of people.
Each of you has been assigned to a small group (5 students each group) focused on an at-risk population. My group will be working on:
• Young adults aging out of foster care (ages 18-25)
1. Read the handout on defining at-risk populations (separately attached).
2. Explore the Healthy People 2030 https://health.gov/healthypeople/priority-areas/social-determinants-health website which explains the framework for understanding Social Determinants of Health (SDOH).
3. There are five key elements to the SDOH: Economic stability; Healthcare access and quality; Education access and quality; Neighborhood and built environment; and Social and community context. I AM ONLY RESPONSIBLE FOR SOCIAL AND COMMUNITY CONTEXT in my group.
4. Examine the challenges your at-risk group (young adults aging out of foster care ages 18-25) faces within the SDOH elements.
5. Use sociocultural theories to explain why this group is marginalized or vulnerable.
6. Identify ways we can advocate for positive social change.
SDOH & Social and Community Context • Describe challenges this population faces with social and community context including neighborhoods and built environments. Give specific examples.
• Explain how we could advocate for this at-risk group to help make positive social change.
• Include in-text citations and references to support your facts on each slide
