Identidy one communtiy program in your community or state that is designed to address health disparities. Choose a program you are unfamiliar with or that addresses a health topic you are not very knowledgeable about. Describe the program, how and wehn it was started, what its target priority is, and its current activities. Provide a brief evaluation on the program. Is the program’s methodology valid? Is its scope sufficient? What is the current and future sustainability of the program? What kind of information can you find on its effectiveness? What measures would you use to determine this program’s effectiveness if you had the chance to work with this program?
Paper should be a minimum of three-pages (double-spaced and 12-point font) and should follow APA formmatting guidelines.
