Section 1: Make an observation: Client and Setting (1-2 paragraphs)
*Use clinical language when describing the client. This section should be written as a clinician summarizing a client, not as a media review.
Provide a descriiption of the client and the setting.
Demographics, family history, when did the problem start, etc.
Identify the proposed diagnosis and symptoms.
Section 2: Ask a Question: Research Question (1 paragraph)
Identify the problem behavior (the problem behavior is your dependent variable).
Provide a brief descriiption of how the problem behavior was determined and how you arrived at the research question.
State the research question (PICO)
Section 3: Gather Information: Literature Review (3 paragraphs)
A review of the published the treatment of the problem behavior (dependent variable).
It should be specific and tailored to your research question.
It should set the theoretical foundation for your project.
Discuss previous studies/interventions.
Evaluate the appropriateness of the articles you have chosen based on client culture, needs, and accessibility.
A minimum of 4 research articles.
Section 4: Hypothesis (One sentence)
Directional hypotheses for all of the key variables in your model.
Your hypothesis should be theoretically justified.
Section 5: Outcome Measures: Test the hypothesis
Use your dependent variable to create a treatment goal (one sentence)
Utilize the SMART goal format.
Create 2 objectives for the client (two sentences)
A descriiption of how your dependent variable was operationalized. (1 paragraph)
Provide a brief descriiption regarding the intervention that will be utilized. This information should be relevant to the literature review previously provided (1 paragraph)
Section 6: Draw a conclusion: Outcomes (1 paragraph)
Provide a review of the outcomes of the treatment goal:
How often will your goal be reviewed?
How will you determine if it is successful?
What other interventions or changes should be considered it the client is not making progress?
