TASK 1 – CONDUCT A CURRENT SITUATION ASSESSMENT The first step in developing an educational intervention for an existing issue or problem of a specific disease process and patient/client population is to define the current situation and the relevant components that affect it. COMPLETE the CURRENT SITUATION ASSESSMENT WORKSHEET: (ATTACHED). Based on the answers from the worksheet, WRITE 350 word-summary of the assessment. TASK 2- IDENTIFY THE TARGET PATIENT POPULATION: The next step is to gain a better understanding of who comprises your target population. A simple definition of the patient population is everyone who is affected by the issue or problem relating to the disease process. Identifying other key characteristics of the population will help to narrow the target population, determine how to engage them, or provide the reason for the health outcome disparity. The goal is to narrow the patient population down into more defined group. EXAMINE the patient population by looking for commonalities that are shared by all the patients. Consider the following: a. Key demographics, including age, income, race, education, housing, geographic area, etc. b. Key cultural and/ or lifestyle aspects, including living conditions, beliefs, religion, values, attitudes, lifestyles, etc. c. Key health identifiers, including generic health trends, tendencies toward obesity, etc. PREPARE a 175-word summary of the patient population that is affected by the identified issue or problem. Address any of the above characteristics that are shared by the population. TASK 3 – CONDUCT A NEEDS ASSESSMENT The purpose of the needs assessment is to identify and understand the learning needs of the chosen patient population. Refer to the CURRENT SITUATION ASSESSMENT and the answers you provided,regarding the current outcomes from the current patient education practices, the desired outcomes, and the differences you identified between the two. IDENTIFY the learning needs of the patient population by considering the following questions: 1. What is the “desired learning outcome we want to achieve,” and why? “What is the problem or opportunity to be addressed?” (Dickerson, 2014). 2. For the intended patient population, what is the potential difference “between where they are now and where they need to be” to achieve the outcome? (Dickerson, 2014). 3. What is the specific need for the patient population to eliminate the difference? What evidence “demonstrates the knowledge, skill, or ability needed” to eliminate the difference? (Dickerson, 2014). 4. How might we improve nursing practice and educate the patient population to address their issue or problem? WRITE a 175 – word detailed summary of this identified need that is not being addressed by the current patient education standards. TASK 4- CONDUCT A SYNTHESIS OF CURRENT LITERATURE. RESEARCH current scholarly information on the identified disease process for current best practices for prevention, management, and treatment. RESEARCH best practices related to your patient population considering identified commonalities and barriers to learning, disease and lifestyle management, and prevention. EXAMPLES of information that you will[ want to extract from your literature review are: *Evidence-based best practices *Best practices for managing and or prevention of the disease process *Best practices for educational interventions. REVIEW 15 academic, peer-reviewed sources. Sources should be no more than 5 years old. CREATE a 1,750- word (5 page) literature synthesis. Include a discussion of the following: 1. Management and Prevention – Evidence-based research found on current best practices for disease prevention and management to improve patient outcomes. 2. Education – Research found on best practices for education interventions that will provide the best learning outcomes for your target population. 3. Population – Research found for best practice interventions for education and disease management with consideration of the social determinants of health identified for your patient population.
