The only difference from the example below is that my made up case is for a patient if the LGBT community.Example:I chose to do my post on a patient who is at risk for a Hospital Acquired C. Diff Infection.Debbie Smith is a 74 y/o female with acute stomach pain related to diarrhea as evidenced by grimacing and pain rating of 8/10. Pt reports loose/watery stools 4-5x a day with accompanying nausea and stomach cramps. Pt was admitted 3 days ago for a UTI. She states new symptom onset was yesterday. Patient states she “does not think she ate or drank anything unusual since being in the hospital” that may have caused the diarrhea. No Hx of abdominal surgeries. Pt currently being treated with Cephalexin for her UTI. Reports feeling bloated. Abdomen appears slightly distended. Abdominal tenderness reported upon palpation of RLQ. Hyperactive bowel sounds upon auscultation. Skin is clean, dry, and intact.Elevated temperature of 100.3. BP Normal: 125/60. Administered 4mg PO Loperamide and 600mg PO acetaminophen as ordered by Physician. Goals for now include to reduce pt’s pain and to get her hydrated.Pt resting comfortably in bed, call light placed within reach. Will follow up in 1 hour and continue to monitor vital signs.
