HPCC 5318: Abnormal Behavior: Treatment Planning and Case Management Assignment 1: Case Study Available points: 100 This assignment has multiple purposes. First and foremost, this assignment is designed to familiarize you with applying principles and models of assessment, case conceptualization skills, relevant theories and community resources to develop an individualized treatment plan. A variety of resources are available to you as a distance student at TTUHSC. The student will research, analyze and develop a case study that relates to your program of study. If you are not currently working, you can make up a story/mock case. Style: Use APA style. Submission must be typed with 1-inch margins on each side and top/bottom. Use the Times New Roman or Arial font with a size of 12. Double-space your work. Check the TTUHSC library website for information on APA style writing. Length: 4-8 pages (as appropriate) Case Studies that do not conform to these standards will not be accepted. Directions: Your case study is should describe a fictitious or mock case. Include the following types of information: • Background/Assessment Information • You may write about: Client name, age, race or ethnicity, marital status, occupation, grade in school, members of household • Initial/presenting concern, current difficulties, physical and psychological health, educational and occupational history, family and home background • Testing history (if available), DSM diagnosis (if available) • Strengths and Resources • Include the positive things going for the person that will help them address the problems and needs they are experiencing • These can include concepts related to personal, familial, intellectual, financial, social, spiritual, physical, medical, or affective supports • Theoretical Conceptualization • Explain how the client developed the presenting concern and is dealing with the concern(s) from a theoretical perspective (., psychodynamic, CBT, person-centered). • Treatment and Monitoring Plan • Treatment plan & goals for counseling using your theoretical conceptualization • Theoretically consistent goals for counseling, as well as information about how your theory helps explain and justify the counseling goals you have established • Measurable long-term goals and short-term objectives relevant to the identified problems. • Interventions that are relevant to the short-term objectives, and treatment recommendations within the continuum of care that are appropriate to the severity of client’s symptoms. • Types of evidence you will use, or have, that demonstrates progress towards client’s goals • Counseling techniques and/or strategies used to help client progress towards meeting objectives and goals that are internally consistent with your guiding theory. • Discuss the role of supervision in helping you address this case • Referral/Service Coordination Plan • Provide three potential referral sites for the case presentation. Students will need to list the name of the referral, and provide a detailed description of how the referral will serve the interests of the client under discussion for the case presentation
