At first, Katie is disconnected from the conversation and later quickly becomes upset and loses her temper at the line of questioning and her mother’s version of events. Symptoms of Disruptive Mood Dysregulation Disorder may include: severe temper outbursts manifested verbally and/or behaviorally, including physical aggression toward people or property that are grossly out of proportion in intensity or duration to the situation or provocation. The temper outbursts are inconsistent with the developmental level. The mood between temper outbursts is persistently irritable or angry most of the day, nearly every day, and is observable by others. This contrasts with Intermittent Explosive Disorder in which there is not a persistent negative mood between temper outbursts. [MOTHER:] Since she was 4 or 5 years old, we thought she’d grow out of them but they occur just as often now as they did when she was younger. [OFF CAMERA:] Describe the tantrums. [MOTHER:] Well, Katie do you want to tell the doctor about your tantrums…Is it okay if I tell the doctor about your tantrums? I’m only doing this to try to get help for you, okay? Katie has asked me several times why this happens and she asked me to get help for her so. Well, Katie gets embarrassed after she has had a tantrum, she cries and feels awful about it and like I said she’s had them since she was younger, she’ll suddenly fly off the handle at the smallest things and yell and throw things and hit and punch. She destroys walls and doors, lamps whatever is close by. We used to have a dog and two cats but we had to give them away because Katie would kick them even though she loved them dearly. [OFF CAMERA:] Um, hum, how often did the tantrum happen? [MOTHER:] Oh, I would say sometimes only two a week but some weeks as many as 5 or 6 times. [OFF CAMERA:] Okay and is there ever a full week that Katie does not have a tantrum? [MOTHER:] Over the years maybe a week or two here or there but it’s usually every week. [OFF CAMERA:] Katie does that sound accurate what your mom is saying? Is it pretty embarrassing? [MOTHER:] Go ahead and tell the doctor what you think and I correct reporting what your tantrums are? [OFF CAMERA:] So every week, let me make sure I understand this, so every week there are 2 to 6 tantrums in a week and that started back when you were 3 or 4 years old, is that right Katie? Okay, thank you Katie. Mrs. Thurston do you ever notice that Katie has periods of having depression or periods of anxiety? [MOTHER:] No, Katie is basically a happy child, like I said she’s bashful at first but she warms up quickly. [OFF CAMERA:] Uh, huh, good, good, Katie do you feel like talking with me now? [KATIE:] About what? [OFF CAMERA:] Can you tell me what kind of things happen that will start a tantrum. [KATIE:] I don’t know, I forget. [OFF CAMERA:] Do you remember, during the tantrums can you remember what happened during the tantrums or do you forget… [KATIE:] I can remember. [OFF CAMERA:] Uh, huh, so your mind doesn’t go blank. [KATIE:] No. [OFF CAMERA:] Oh, okay, when was your last tantrum? [KATIE:] Yesterday. [MOTHER:] I believe it was two days ago. [KATIE:] Two days ago. [OFF CAMERA:] Do you remember what set you off? [KATIE:] I was building a bird house and the roof wouldn’t fit. [OFF CAMERA:] Um, hum a bird house. [KATIE:] Yep. [MOTHER:] Don’t say yep. [KATIE:] I mean yes. [MOTHER:] Thank you. [OFF CAMERA:] How long would you work on the tree house? [KATIE:] Bird house, it’s a bird house. [MOTHER:] Katie be polite, be polite. [OFF CAMERA:] So how long had she worked on it? [MOTHER:] She was almost finished with it, an hour or so. [KATIE:] No I wasn’t almost finished, I haven’t even started painting or putting on the roof of anything, you always make it sound like I’m doing better than I am and you I’ve ruined it. You lie to make people think I’m doing great when I’m not, I hate when you lie to everyone. [MOTHER:] Katie stop it. [KATIE:] You lie to the doctor that I’m a good person. [MOTHER:] I did not lie, you are a good person. [KATIE:] You never listen to me that I’m having trouble, you pretend that everything is fine and it’s awful, everything that I do is awful.
