You are going to describe what you would do if you were to do this in a community.
The proposal must be between 5 to 8 pages, excluding any graphics (maps/graphs/pictures), and requires the following information:
Identify the population and health topic
Assess the needs of the priority population
Set the goals and objectives for the program
How will the goals and objectives be achieved
Describe your implementation – how would you do this?
Describe how will you implement the interventions
Describe how you would evaluate the results. How will you know that your intervention improved the health of the target population?
The proposal must be between 5 to 8 pages, typed, double-spaced, not including references, appendices. Follow the APA Publication Manual (7th Edition, 2019 for proper citation, quotes, tables, figures, footnotes, appendixes, text and reference list, etc.
