A comprehensive, strength-based treatment plan
I = Individual Counselor (This section is about you in regards to your strengths, concerns, or weaknesses/challenges that you bring to the therapy room with this specific client.)
C = Contextual Assessment (This section is about the entire context of the client which includes but not limited to age, family, economic status, education, gender, past issues, experience with therapy, etc.)
A = Assessment and Diagnosis (What is your assessment of the client? What assessment tool would you use? Use the DSM-5 to provide a diagnosis. Support your diagnosis with actual verbiage, codes etc. from the DSM-5.)
N = Necessary Level of Care (Based on what you read about the client, what level of care is needed?) (office visits, group work, outpatient, hospitalization etc.)
S = Strengths (What strengths did you read about the client?)
T = Treatment Approach (What is your approach to treatment? Theoretical Foundation?) (Share one theory you would use?)
A = Aim and Objectives of Treatment (SMART Goals- Specific, Measurable, Achievable, Relevant, and Time-Bound) (Share one SMART Goal)
R = Research-Based Interventions (Share one research-based intervention)
T = Therapeutic Support Services (Share one support service you would share with client.)
