SOCIAL WORKER: Hi, Glenn. It’s great to meet you. My name is Rhonda. And I’ll be
your social worker today. I’m glad we were able to make a telehealth session work. So
what brings you here today?
MALE SPEAKER: Well, my captain referred me. I’m a firefighter. And I froze on two
calls.
SOCIAL WORKER: Can you tell me about them?
MALE SPEAKER: It was an apartment fire. I mean, there were a lot of trucks there. So
no one noticed, except my captain. He picked up on it. He changed my assignment so I
didn’t have to go into the building.
SOCIAL WORKER: And the second time?
MALE SPEAKER: It was a car wreck, someone trapped inside the car, blood. Again, we
were late to the scene, so I was able to hide, but–
SOCIAL WORKER: That’s OK.
[SCOFFS]
MALE SPEAKER: My captain sees all, you know? He caught me hanging back.
SOCIAL WORKER: Has this happened before?
MALE SPEAKER: No. I’ve never been afraid before. You can’t be.
SOCIAL WORKER: I bet it’s hard to always be fearless. Something change?
MALE SPEAKER: So– all right. You know the school shooting a few weeks ago? I was
first on the scene.
SOCIAL WORKER: It must have been difficult.
MALE SPEAKER: Yeah, it really was. Kids lying there, not moving.
SOCIAL WORKER: How does it make you feel when you remember it?
MALE SPEAKER: It doesn’t haunt me so much. I mean, I’ve seen some crazy stuff. You
know? But when the calls come into the station, the minute I hear the alarm, my heart
races, my head gets cloudy.
SOCIAL WORKER: And then what happens?
MALE SPEAKER: I don’t want to be afraid. There, I said it. I just want to get back to
work.
Assignment: Application of Cognitive Behavior Theory to a Case Study
This week, your theoretical orientation is cognitive behavior theory. You will use the same case study that you chose in Week 2 (Sam Franklin) and have been analyzing in this course. Use the Analysis of a Theory worksheet to help you dissect the theory.
In this Assignment, you prepare a 5-minute video case presentation. It is common to present a case analysis in multidisciplinary team meetings or with your supervisor and colleagues. This assignment offers you an opportunity to provide insights and perspectives to a case.
To Prepare
• Use the same case study that you chose in Week 2. (Sam Franklin)
• Use the Analysis of a Theory Worksheet to help you dissect the theory. Use this tool to dissect the theory, employ the information in the table to complete your Assignment, and then keep it to add to your Theories Study Guide in Week 11.

Submit A Transcriipt so that I can conduct the video portion with my classmate as the client.
• Note: Your video should be no longer than 5 minutes.
• Also Note: This Assignment is an entirely oral/video presentation—there is no PowerPoint element to this presentation. Although this is a professional presentation, it is not a formal presentation as in a speech. Imagine that you are discussing your case with a group of peers in a meeting. For example, if you stumble a bit, don’t feel you have to record yourself again.
• You can have notes in front of you to help you remember your points.
• However, your video does need to be professional. In other words, dress professionally and conduct yourself as if you are in an agency setting with colleagues and supervisors. Be sure to maintain eye contact as if you were speaking in front of your colleagues. Finally, be sure to record yourself in a room that is quiet and where no one interrupts you.
Your video presentation should include the following:
• In 1–2 sentences, identify and describe the presenting problem.
• In 1–2 sentences, briefly define and conceptualize the problem from a cognitive-behavioral theoretical orientation.
• Formulate two assessment questions that you will ask the client to better understand the client’s problem. Remember, the assessment questions should be guided by cognitive-behavioral theory.
• In 1–2 sentences, identify two goals for treatment. Again, remember, the goals should be consistent with cognitive-behavioral theory.
