Identify a situation where you believed your client’s safety was at risk because of improper communication during any of your clinical placement in the practical nursing program. Use evidenced based literature to support your rationale that the situation was unsafe. 2. Using the Patient-Safe Transformational Model of Communication, identify one (1) context and the two (2) risk factors that affected the communication in the incident. Explain how the context and risk factors affected communication in the situation that was identified. Use evidenced informed literature to support your understanding of the one (1) context and two (2) risk factors. 3. Discuss three (3) Patient-Safe Communication Strategies that should be implemented for the situation to create Transformation. Selected strategies must be strongly linked solutions for the identified problems in the situation. Evidenced based literature must be used to support the rationale for each of the identified strategies. Note: It is suggested that students use headings to clearly identify in each of the above (three) areas.
