Assignment: Evaluation Summary & Implementation Plan Purpose: This final assignment allows you to demonstrate how you have developed an innovative solution to an issue relevant to at least one of the social determinants of health as identified in Healthy People 2030 affecting a target population. It will also reflect the synthesis of information gathered from research and exploration of health and social needs of your target population during the design process. Please address the following four points in your paper. Summary of the history and current impact of the problem you have identified within the target population you selected for your innovative solution. (2 points)Based on the interdisciplinary research of the problem identified, explain how it aligns with one or more of the five domains, including the specific objective(s) of the social determinants of health. (5 points)Economic Stability,Education Access & Quality,Healthcare Access & Quality,Neighborhood & Built Environment, and/orSocial & Community ContextBased on findings during the asset mapping and gap analysis component of the design process, relate how your innovative solution addresses the top needs, interests, or concerns within the target population. (5 points)Implementation Plan – Analysis of evaluation results and further review on prototype – Is your innovative solution sustainable to move forward or revision/alternative necessary? If you believe your plan could move forward, what would be the next steps to make this a reality?
