Answer three questions based on the given scenario
Each response should be no more than 500 words (+/- 10%)
Scenario.1
You are the midwife caring for Jess who is having her second and third babies as she is pregnant with DCDA twins. She is 33+6 weeks pregnant and has presented to your tertiary referral hospital having contraction type pains. She has had an otherwise low risk pregnancy and is GBS unknown at this stage. Her first baby was born vaginally at term.
Question.1
Discuss the midwifery care you would provide to Jess whilst also discussing the clinical guidelines associated with this scenario and critically analyse the evidence that supports this clinical care.
Scenario 2
Jess continues to experience increasingly painful contractions. It has been determined that her babies are presenting as Twin 1 cephalic and Twin 2 breech. She is starting to ask for pain relief for her contractions.
Question.2
Discuss the pain relief options for Jess including the pharmocology associated with medical forms of pain relief and current evidence surrounding the use of non-pharmacological and pharmacological forms of pain relief and how this is relevant to Jess’s situation.
Scenario.3
Jess has decided that she would like an epidural for pain relief and is encouraged to go to the toilet to empty her bladder prior to the insertion of the epidural. She calls you from the bathroom ans states that she thinks her waters have broken and there is something ‘weird’ between her legs. You determine that Jess has spontaneously ruptured her membranes and that twin ones umbilical cord has prolapse and is now sitting outside of the vagina.

Discuss your clinical care at this time based on current clinical guidelines and woman-centered philosophy. What are your main considerations? What evidence supports the clinical guideline for managing this emergency situation.
