Cecil is a 72 year old man living at home with his 64 year old wife, Ann. He retired 1 year ago from his job as an engineer. Cecil’s medical history includes type 1 diabetes, which he manages himself. Ann continues to work fulltime in her job as a primary school teacher.
Ann has brought Cecil to the GP as she is concerned about his mood and behaviour which has changed over the last few months. He wakes up at 5am and can’t get back to sleep (he normally sleeps until 7am) He reports struggling to adapt to retirement and feelings of being unmotivated to do the hobbies he normally enjoys doing, and doesn’t feel like being social anymore.
He used to meet weekly with his workmates at the pub but has not done this for the last month. His wife reports concerns about this memory as he is getting forgetful and missed attending a couple of appointments as he had forgotten about them.
Cecil’s BGLs have been up a little in the past 2 weeks, and Mary is unsure if Cecil is eating more than normal during the day or not taking his insulin correction.
Cecil acknowledges he has forgotten the appointments but does not feel that there is an issue with his memory.
Assessment Outline:
