Think about your place of work and discuss the following. 1. Discuss the management and support of a patient like Mrs Djokic if she was to come to your unit (acute through to community). How well is your workplace (or you) able to cope with an elderly patient who has both an acute condition associated with two chronic conditions (diabetes and dementia) 2. What is one thing that your local environment could improve to make the management of Mrs Djokic less traumatic (for her and her family) and also prevent a worsening of her chronic conditions? This could be communication, service provision, support education think laterally.
