Health history
You must complete the following in the health history data collection tool with a “patient”, the patient can be a friend, family member, etc.
Biographical Data
Reason for Seeking care (use their most recent illness, can be anything that caused them to see a doctor or required treatment, treatment can include analgesic/antipyretic at home, prescription medications, or rest)
Present Health (PQRSTAU) about the reason for seeking care
Past Health History and Current Medications
Family History
Review of systems (Just review the system involved with the reason for seeking care NOT all systems)
Submit the completed health history document as above into the assignment folder.
Discussion Forum Instructions:
Case Study 1: Due Week 3 initial post due by the Wednesday and response post by Wednesday of Week 4
Create a new thread.
Write at least a 1-2 paragraphs including the following information:
Based on the health history interview you conducted on your “patient” identify 1 thing that went well and 1 thing that didn’t go well with rationale. Provide examples from your interview and reference your textbook.
Also provide a brief (2-3 sentences maximum) regarding your patients “reason for seeking care” to provide your peers with context.
Consider discussing the following issues:
Therapeutic questions that worked well or those that didn’t
Areas that were challenging to address within the review of systems and why you feel that they were a challenge.
How did the physical setting impact your interview positively or negatively.
