You will be assessing the current education material that is given to the patient population. You will be designing 2 different questionnaires that are meant to gather sufficient information to guide the creation of your education intervention. The FIRST questionnaire will be directed at the patient population, their families, and their caregivers. The SECOND questionnaire will be for those individuals who comprise the population’s care team, such as patient educators, case managers, physicians, pharmacists, physical therapists, dietitians,or other individuals participating in the direct care of the identified population. TASK 1 – Current Patient Education Material Assessment: For this task, you will perform an assessment of the current patient education material that is provided for the targeted patient population using the Patient Education Material Assessment Tool (PEMAT) from the Agency for Healthcare Research and Quality (AHRQ). PEMAT is a systematic method to evaluate and compare the understandability and actionability of patient education materials. Using the PEMAT Auto Scoring Form, EVALUATE the patient education material by following the instructions contained in the tool. NOTE: There are 2 different forms, one for print materials and one for audiovisual materials. WRITE a 350-word summary of the state of the current patient education material explaining the most explicit deficiencies. Task 2 – PATIENT SURVEY QUESTIONNAIRE A key element to identifying the learning needs of the selected patient population is to interview the group members directly. DEVELOP questions that you will ask individuals within the targeted patient population. These questions should focus on the key issue or problem you identified, provide a clear understanding of the population’s learning needs, identify learning barriers, provide support for potential solutions, and seek feedback on the current education material provided in the clinical setting. Your questions will also be directed to family members and anyone else who is involved in the care of the patient (i.e., caregivers). USE the Patient Interviews document to assist you in developing questions for the interviews. You do not want to overwhelm the patient with questions. Keep interviews to approximately 20 questions. TASK 3 -Interprofessional Collaborative Care Team Survey Questionnaire The interprofessional collaborative care team members can provide invaluable feedback in terms on what patients need to know from their perspective and role in the team. CONSIDER THE FOLLOWING AS YOU CREATE THE QUESTIONNAIRE: *What are their (pharmacist, dietitian, case manager, etc) top priorities for patient education for the identified population and recommended solution? *What does the facility offer as educational material for patients and families with this disease process? a. Review the current education materials with those you are collaborating and get their opinion on what is missing from it and how to improve it. *How is the information delivered to the patient? *What information/education is not currently utilized/available that would be valuable for this population? *Are there any breakdowns in communication among the care team members? *What could be done to improve communication and collaboration among team members?
