Clinical/practice problems in need of resolution always start with the patient and concerns regarding experiences, the effects of therapy, the use of diagnostic tests, the prognosis and/or etiology of disorders, or the environment in which care takes place.
Identify a clinical/practice problem that is within your future advance nursing role (acute care NP) scope.
Describe how you know it is a problem and the significance of the problem. This a a great opportunity to use benchmark data to support your position.
You should be able to identify and describe the clinical/practice problem in 1-2 paragraphs.
A problem statement clearly describes the problem to be resolved. A problem statement should identify the issues and problems in the current situation, describe the context of the problems, define the magnitude of the problems using statistics and other epidemiological data as appropriate, describe the impact of the problems on the population, and provide solutions. A well-constructed problem statement provides focus for the team. The five Ws— who, what, where, when, and why— are a good tool to use to focus on the problem statement. Consider Rudyard Kipling’s poem “The Elephant’s Child” : I keep six honest serving-men (They taught me all I knew); Their names are What and Why and When And How and Where and Who.
Who —Who gets affected by the problem? Specific groups, organizations, stakeholders, and so on?
What —What are the boundaries or context of the problem, for example, do they involve the organization, workflow, geography, customers, segments, and so on? What impact does the problem have? What would happen when it is fixed? What would happen if it is not fixed?
When —When does the issue occur? Where —Where is the issue occurring? Only in certain locations, processes, products, and so on?
Why —Why is it important that we fix the problem?
