Information Gathering: In this phase of the case study, you are required to capture as much information about the client as possible. As you read the case, document the following information as provided. Client background information—Discuss personal information about the client such as ethnic origin, age, and gender. Patterns of Substance Use and Addictive Behaviors—Explain the client’s current and past history with substance use or addictive behaviors. Also, explain the frequency, the amount consumed, and the length of time of the active addiction. Level of dependence—State whether or not you think the client has an addiction problem Family background and social support—Discuss family and social relationships that may influence addictive behaviors. Vocational and Financial background information—Explain the client’s financial history and current or past employment opportunities Addictive behavior related to health problems –state the client’s physical health problems and how addictive behaviors plays a role in the client’s current health conditions Mental Health Challenges—Discuss whether or not another mental health-related problem exists. Stage of change—State whether the client is ready or not ready to change addictive behaviors and why
