Valentina is 83, uses a cane, and is slightly confused. She has been diagnosed with Congestive Heart Failure. She is currently taking Digoxin, Lasix, Metoprolol, Potassium and Aspirin. Their new home is one story, with two bedrooms and one bathroom. You are the home health nurse assigned to monitor Valentina.

Lucia greets you at the door and you enter the home.
As you say hello to Valentina, she shares with you she now has the “sugar”
disease. Then, as Valentina gets up to go to the bathroom you notice she is
using a cane and has an unsteady gait. You decide to do an environmental scan
for safety prior to completing the home visit.
Jaylen is also in the room sitting on the couch with a male you haven’t met

“went a little crazy.” Additionally, he shares his recent diagnoses of bipolar
depression. He confided with you that he is worried his bipolar disorder cannot
be treated and he will never return to school.
After you collaborate with Jaylen and Lucia on a plan to help Jaylen get back to
