Child’s History
Present
Write one paragraph in which you give a physical description of the child as well as information on his/her performance, ability, and achievement. Mention areas of strengths and weaknesses. Mention the behavior(s) that needs to turn into a strength.
Past
Write one paragraph in which you include any important information about birth and early childhood such as normalcy of birth, evidence of child’s pace of development in locomotion, speech, and social areas. Mention any health problems that the child might have.
Family History
Father and mother: ages, occupation, health, education. Parental availability in the house. Child caring situation. Mention of divorce, death or any other major events related to the parents. Attitudes of parents toward the children or child. Sources of anxiety; feelings about discipline consistency and agreement between parents about the children. Siblings; list in order of age all the siblings, name, age, sex. Include information related to the child you study.
Current School Procedures
Methods used by the teacher up to now; describe the methods by which the present teacher has tried to work with student toward meeting his/her educational and social/emotional needs.
