Step 2: For this discussion, select one of the health care delivery settings discussed in the assigned reading (e.g. home health/hospice, long term care, the VA system, retail/nursing clinics, community health centers) and consider ways in which the setting could incorporate aspects of the PCMH model. Specifically address access, quality, and cost. Finally, discuss the nurse’s role in advocating for these changes. If possible, share your own experiences and example of this model (or related principles).

According to the Center for Disease Control (2021), “The patient-centered medical home (PCMH) model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system”.

I work in long-term care and can see the benefits of implementing this model. Quality is improved as the model is patient centered. There is also a great amount of focus on safety and quality which would improve patient outcomes. Access is increased because of the team focus that the PCMH model provides. Healthcare costs would be reduced as more patients would be receiving preventative services which saves money in the long run. The nurses’ role in advocating for these changes is to influence policy. “Nurses can have a profound impact on policymaking by using their knowledge to frame and define health policy alternatives” (Mason, et, al., 2020, p. 13). I could share the benefits of this model with my team and possibly shift the model we currently have toward it.

As a nurse, I recognize the importance of the patient-centered medical home (PCMH) model in improving access to care, increasing quality, and reducing costs. According to the American Academy of Family Physicians (AAFP), “PCMH is an approach to providing comprehensive primary care for children, youth, and adults, with the goal of maximizing health outcomes and patient satisfaction” (AAFP, 2017). One way that my workplace, a community health center, could incorporate aspects of the PCMH model is by offering coordinated care teams that work together to address the multiple needs of patients. This can help to improve access to care by providing a central point of contact for patients and promoting continuity of care.

In terms of quality, the PCMH model emphasizes patient engagement and shared decision making. This can be achieved by using tools such as patient portals, which allow patients to access their medical records, communicate with their care team, and manage their own health information. As the Agency for Healthcare Research and Quality (AHRQ) states, “Engaging patients and families in care is a critical component of a patient-centered medical home and can lead to improved health outcomes” (AHRQ, 2020). By implementing patient engagement strategies, our community health center can ensure that patients are active participants in their own care, leading to improved health outcomes.

Finally, as nurses, we have an important role to play in advocating for the implementation of the PCMH model in our workplaces and beyond. This includes staying up-to-date on the latest research and guidelines related to the PCMH model, educating our colleagues and patients about its benefits, and working with our organizations to make necessary changes. As the National Council of State Boards of Nursing (NCSBN) notes, “Nurses have a unique role in promoting and implementing patient-centered care” (NCSBN, 2019). By taking an active role in advocating for patient-centered care, nurses can help to improve the health outcomes of our patients and communities.
