Identify different factors from a biopsychosocial framework. Identify where psychologists may identify gaps or areas of intervention to help the patient. Develop some potential treatment goals for work with your patient. Each case study should be able 2-3 paragraphs of a response.
1) A 17 year old man has been referred to you following a new diagnosis of Type 2 diabetes. His primary care physician is worried that he has had several follow-up visits where his A1C has gotten worse instead of better, and patient is unable to identify what is getting in the way after his basic interview. Doctor reports that he obtained the following information:
* Patient is fully responsible for own diabetes management.
* Patient has no prior involvement in mental health care and is not convinced that behavioral health involvement is what he needs. However, on his depression screen, he is endorsing several mood related symptoms.
2) In the hospital, you are consulted to evaluate a 12 year old female patient who presented to the hospital complaining of various GI symptoms. Medical team is unable to find a medical explanation for symptoms, and family is getting increasingly frustrated that medical team is unable to provide answers and that they feel that team thinks “it’s all in their child’s head.” Family requesting “re-running” all GI testing and a second opinion.
•Patient has a prior history of generalized anxiety disorder, and received a few sessions of therapy, but felt it “wasn’t helpful.”
•Family reports that symptoms are getting in the way of life- patient has missed a lot of school and spends a lot of time in bed, becoming more isolated.
• Medical team asks you to “deal with this family, because they are unmanageable”
