chose.
2. Write a summary on the
high points of the integrated treatment plan. Be sure to include all aspects of
mental health and wellness including any natural supports you can think of for the client or patient.
3. Describe steps that can be implemented to increase adherence or motivation for patients to the integrated care plan.
4. Describe how the following professionals on the team will work together to integrate care including how to keep the patient engaged: physician lead, care manager, and behavioral health professional.
