“Stress and Adaptation” topic in relation to the patient we were assigned. Pt. information: 75 year old hispanic female with an extensive history of psychiatric issues including anxiety, bipolar disorder, dementia (primary diagnosis), paranoid schizophrenia. Pt. was admitted for type 2 diabetes with hypertension; additional history includes osteoporosis of left leg, hypothyroidism, UTI unspecified, and anemia. Only pain medication prescribed is Acetaminophen 325 mg, PRN (as needed). Pt is AAOx2; alert to person and place, otherwise confused. Any slight movements provoke a grimacing and “ouch” reaction. Pt is uncooperative for eating and being interviewed. Do with that information what you will, some of it should be included, though. I will attach what I’ve written, which frankly, is straight up BS because it was done in 40 min. I will also provide instruction paper.
