Hospital readmission is a healthcare crisis, especially for individuals over the age of 65 years old. Patients over 65 years old experience disparities in care because of hospitalization (Coatsworth‐Puspoky et al, 2021). My pilot study aims to decrease readmission rates in home health patients to improve outcomes and decrease health disparities and inequities. I have chosen to use Kurt Lewin’s 1977, Theory of Planned Change (TPC) model as the framework for the study. Lewin, who know as the father of social psychology developed the framework. The framework fits the inquiry (PICOT). The PICOT question: Can interprofessional (P) case conferences weekly (I), compared to no case conferencing (C) decrease 30-day readmissions (O)?
