Demographic and Epidemiological Data help to describe the health and wellness of communities and population groups. This assignment gives you an opportunity to locate and analyze key demographic and epidemiological data for your selected County using a template to guide the assessment process.
Note: This part of the Community Assessment, along with Parts II, III, and IV, will be completed for the county you selected in the county sign up conducted previously. This will, most likely, be a different county than the one for which you conducted your windshield survey.
Instructions:
Download the Community Assessment Part I Template and read the guidelines carefully to ensure you meet the assignment requirements.
Use the resources included to gather demographic and epidemiological data for your county and record that information in the Demographic Data and Epidemiological Data sections of the template.
Analyze the data and complete the Data Analysis section of the template.
– Include a 2-paragraph summary,
– identified health promotion topics,
– a social determinant of health.
– Include supporting evidence.
Use a third person view for the summary to maintain objectivity.
Use proper APA citations and references throughout.
