This assessment item consists of conducting, analyzing and writing the history component of a Comprehensive Geriatric Assessment that you will conduct with an older adult. The case review will include the following components:
• Identify what matters most to the older adults. (you can google what matters most conversation to see what it is)
• Identify the most pertinent health concerns for this person. While considering their medications, mobility, mentation and multi-complexity.
• Review the older adult’s medical and surgical history, as well as their allergies and medications.
• Review their social history, housing and functional status to understand where they are situated.
• Identify the most relevant health problems or significant findings based on your completed history, consider the patient’s goals and self-identified concerns in conjunction with your own findings gained from the history.
• Identify the potential impact of these findings to the older adult and your next steps to support this older adult.
• Describe how you may transfer the knowledge of the Geriatric 5M Framework into your current practice area. Identify any challenges or modifications that may be needed in order to adapt this to your practice.
This assignment is a maximum of 10 pages double spaced, excluding references, title page, and any appendices.
