Case Study A
Illicit drug use among pregnant women is on the rise. Intrauterine opioid exposure can result in neonatal abstinence syndrome (NAS). Newborns exposed to addictive substances during the mother’s pregnancy begin withdrawal as soon as the umbilical cord is cut. Most infants born to drug-dependent mothers will undergo NAS (between 55% and 94%). The severity of presentation is based on the infant’s level of exposure in utero. Of exposed infants, 60% to 80% will have moderate to severe symptoms requiring medical intervention. Infants with NAS may exhibit a constellation of central nervous system, gastrointestinal, autonomic nervous system, and respiratory symptoms (Finnegan & MacNew, 1974; Hudak, Tan, Committee on Drugs, Committee on Fetus and Newborn, & American Academy of Pediatrics, 2012). According to the severity of the withdrawal, the duration of symptoms is anywhere from 6 days to 8 weeks (Finnegan & MacNew, 1974; Patrick et al., 2012). Studies of inpatient populations of infants with NAS report a wide range of length of stay (LOS), from 1 to 122 days; (Burns & Mattick, 2007; Johnson, Greenough, & Gerada, 2003; Seligman et al., 2008).
Clinical practice guidelines for NAS provide evidence-informed recommendations that address the needs of substance-using pregnant women and newborns at risk of NAS. Reducing the incidence and impact of NAS requires immediate action in order to improve the care of affected women and infants. This includes optimizing and standardizing treatment strategies, assessing and managing social risk, monitoring of prescribing practices, and facilitating the implementation of better treatment and prevention strategies as they become available. These clinical practice guidelines provide the framework to inform and support the development of a coordinated strategy to address this important issue and to promote safe, effective care.
Toward this end, an evaluation of inpatient services, both maternal and infant, was designed with the objective to improve NAS inpatient outcomes. An interdisciplinary team was put together to optimize care strategies for substance-using pregnant women and their infants.
