Attached you will find information for two patients including gender, age and medication lists outlined in the clinical simulation document attached in the additional materials. From these medication lists you will work backwards to create 2 separate patients. Create the full picture of the patients (this information is made up) including HPI, Presenting Signs and Symptoms, Abnormal Physical Assessment Findings, Admitting Diagnosis, Medical History, Psychosocial and Socioeconomic Data, Allergies etc. Once you have an understanding of your patient and the condition(s) they are being treated for, complete a Patient Care Flow Map, a formal Nursing Care Plan and a full Medication Sheet for each patient utilizing the posted templates. Turn in these documents in via Blackboard in the clinical assignments tab under the make-up assignment folder. The patient care flow map, care plan, and medication sheet documents are all also attached in the additional materials section.
