Part I: Scenario
You receive a referral from a school regarding a family whose student has missed 5 days of school. The student has an IEP which specifies the student’s classification as emotional disturbance. Additionally, the child’s historic data indicates the child missed 18 days of school last year. It was also noted in the student’s file that the mother has been unresponsive and/or resistant to receive support from the school. Utilizing this information, please develop a family success plan for this student.
Please include the following in your family success plan:
1. A description of your engagement strategy for the child and family
2. A list of community resources to connect and or refer the student to.
Specify the length of time the plan will be in effect, how identified community resources could support the child, along with specific attendance outcomes for the student at the close of the action plan.
