TA, a 26-month-old male with Down syndrome, was brought into the ED by his mother who
reports he has been complaining of abdominal pain x 2 days, with one episode of emesis.
Patient fell out of bed while napping and is now lethargic, sweating, breathing rapidly, and has
low urine output. He has a PMH of atrial-septal defect repair with transient CHF as an infant.
The child is listless, clammy, and pale. Upon exam, the patient is hypotensive, tachycardic,
tachypnic, with signs of dehydration. His abdomen is distended with epigastric bruising, firm
abdomen to palpation with diffuse tenderness, guarding, and rebound tenderness, with a
reducible umbilical hernia
