Explain how Care coordination is important for everyone. Over the life course, everyone will have at least a
temporary need for improved coordination and communication between providers, settings, or
periods of time during and illness. Thus, everyone is at risk for, at a minimum, an episodic
illness with at least temporary needs for care coordination. Effective models of care coordination
continue to emerge but the translation into practice has been slow. Informal caregiving is one
particular example of care coordination that has little empirical research to confirm the presence
of specific attributes of care coordination models.
a. Use the PICOT model (P – patient/problem; I – Intervention; C – Comparison; O –
Outcomes; and T – Time) to create a research question that addresses the specific
attributes of informal caregiving and its value in the coordination of care.
b. Find a research article in a peer-reviewed journal that is less than 5 years old, that
addresses informal caregiving. Write 2-3 paragraphs to summarize the research. Attach
the article to this submission.
