Write a paper on how to achieve a health care culture where continuous care (versus episodic encounters) is the standard, health care professionals must work in collaboration and not in isolation. As such, educational transformation is needed. The framework for need has been set in chapter 1 with an outline of key terminologies. A large body of evidence supports the rationale for transformational change in health care and education. Historical evidence outlines the beginning of this transformation, but there is still much work to be done. Many organizations and agencies have begun to address the need for sustainable change, and perhaps if they all collaborate, the possibility for success exists.
