Assessment : After attending and reviewing the lecture and course material, read the scenario below: Scenario: Alisha McArthur is 20 years of age and is enrolled in her first semester of the Bachelor of Nursing. Alisha has been introduced to the concept of professionalism which includes professional values, knowledge, skills and attitudes that are all used in person-centred nursing care. She has also learnt about professional boundaries that govern professional communication interactions with patients. Beginning in semester one, Alisha is learning foundational nursing skills in a simulated context to begin her journey to become a registered nurse. She has been introduced to the Nursing Council of New Zealand’s (NCNZ) Competencies for Registered Nurse practice. It contains four domains and their associated competencies that she needs to demonstrate while she is a student and when she has qualified as a registered nurse. In Domain One, 1.1, the competence required is: “Accepts responsibility for ensuring that his/her nursing practice and conduct meets the standards of the professional, ethical and relevant legislated requirements”. In Domain three a competence that is required is 3.1: “Establishes, maintains and concludes therapeutic interpersonal relationships with health consumers”. Alisha has learnt how to introduce herself in a professional manner to a patient using the acronym AI2DET. AIDET stands for acknowledging the patient, identifying self and the patient, stating the duration of the interaction, explaining what the interaction is about, enquiring if the patient has any questions and thanking the patient at the conclusoin of the interaction. In order to develop a therapeutic relationship with a simulated patient, in this case, Mr Baird, Alisha has been instructed by theregistered nurse (RN) lecturer to demonstrate her first responsibility which is to meet hand hygiene standards. Hand hygiene is a fundamental skill that all nurses must uphold before any interaction with a patient for safe nursing practice. Alisha has learnt the five moments of hand hygiene and she is keen to demonstrate her abilities to her fellow student, Chloe. She is also keen to practice her therapeutic interpersonal relationship skills with a simulated patient. Chloe asks: “Alisha, I know we are meant to be practicing how to develop an interpersonal therapeutic relationship with a patient but what is the difference between a personal, a social and an interpersonal therapeutic relationship? Why is hand hygiene so important? Are professional boundaries necessary when we speak with patients? How is professionalism linked to the way we maintain hand hygiene practices and speak to patients as we provide nursing care”?
