2.5 Building a personal support network
You are halfway through this course. This is an Assessment of Learning, which is used to evaluate your work based on established criteria and to assign a mark. Your teacher will provide you with feedback and a mark. This Assessment of Learning is worth 17% of your final mark for the course.
There are three tasks in this Assessment of Learning.
This lesson has presented you with many different sources of information to provide support for health-related issues such as physical fitness, nutrition and stress. In this assessment, you will have an opportunity to build on the knowledge that you have learned in this lesson as you put together a presentation to help someone struggling with a specific life issue.

The student is feeling completely overwhelmed by school. They can’t stay organized or get caught up with assignments and their grades are starting to slide. They explain to you that they haven’t been eating well or exercising. Your calculations find that they are above the typical average weight for their age group.
The patient’s family culture is such that the family doesn’t communicate a lot about their personal feelings or struggles. As a result, the student has not been able to ask anyone for support. As the doctor, you need to come up with a plan to help this student and others like them. You will create a presentation to outline this plan in the form of a slide show presentation, brochure, or infographic.
Your plan must cover the following categories:
• Mental health
• SMART goal setting
• Nutrition
• Exercise
• Stress management
Task 1: Identify three goals for the individual
Using the “SMART goal organizer(Opens in new window),” create three different goals for this student to accomplish that will help them get to a healthier place in life. For each goal, you may choose to focus on an area of personal growth, a nutrition goal, an exercise goal or something related to stress management.
Task 2: Compile a presentation that demonstrates your plan to help this individual
Present your plan in a presentation that can take the form of a slide show (such as PowerPoint), brochure, or infographic. Your presentation should have a minimum of ten slides and include a title slide, images, and statistics. You will also need visual images, pictures, or graphs to enhance your presentation.
Your presentation should address the following five areas:
Mental health
Do research on teen anxiety and mental health challenges. Provide information on the kinds of issues that anxiety can cause in a high school student’s life and describe some of the warning signs.
SMART goal setting
List and explain the three goals you have developed for that student using the SMART criteria.
Nutrition
Provide guidelines for healthy eating using the Canada Food Guide and other resources for healthy eating. Support your information with the health benefits of proper nutrition.
Create a sample one-day meal plan for the student, including three meals and snacks. Include the total daily caloric intake.
Exercise
Provide guidelines and benefits of exercise for this student. Connect the information you are presenting to the health benefits for the student. Create a sample 60-minute exercise routine for the student, including aerobics, strength training, and stretching.
Stress management
Provide information on stress management and locate three community-based resources or techniques this student can access to help manage personal stress. Suggest three specific activities or organizations that could help this student reach a healthier place in life.
Task 3: Submit a bibliography that lists the sources used
Use a minimum of five information sources to support your presentation. Compile your sources in a bibliography page, with sources cited in APA format. You may use any citation-building tools, which can be found by searching the internet using the terms “automatic,” “APA,” and “citation.”
