Using a problem or opportunity for change you can identify in the clinical nursing setting, create a plan for change/improvement using the following seven steps. The identified change potential can be real or hypothetical.

3. Select and Analyze Data

a. Identify/list potential resistance to change.

b. How might you work to build consensus around the change?

4. Develop a Plan for Change, Including Time Frame and Resource

a. What is the target system for this change? How will you work to enlist participation from the members of this system?

b. What is the time frame for this change? Break this down into steps with timeline.

c. What resources will be necessary for successful implementation of this change?

d. What operational indicators can you identify that can serve as measures of change implementation success or failure?

5. Identify Supporters and Opposers

a. Who will gain from this change and who may feel a sense of loss?

b. Discuss the aspect of power. Who may feel like they have more power and who may feel they are losing power?

c. How will you, as the planner, mitigate potential negativity surrounding this change and promote positivity?

6. Implement Interventions to Achieve Desired Change

a. What is your role, as change agent, during this step?

7. Evaluate Effectiveness of the Change, and, if Successful, Stabilize the Change

a. How will you determine if change implementation was successful?

b. How will you determine if the change is effectively addressing the problem or opportunity for improvement? What are your indicators?

c. If successful, how will you terminate the change agent role and ensure long-term maintenance of change?
