Download the Plan Do Study Act (PDSA) worksheet, save it to your files, review the document in its entirety, and create a PDSA. The goal of this assignment is to test a change on a small scale either a personal level or in your workplace.
Please incorporate an initial target goal into your PDSA in the form of an aim statement (this goes under Planning). An aim statement is a clear, specific summary of what you plan to achieve. It includes a time element and a target goal.
Personal aim statement example: “Within 5 days, I will increase the total number of sleep hours per night from 7-8 hours by incorporating meditation and yoga each night into my schedule.” In this example, you would track the number of sleep hours per night.
Clinical aim statement example: “Reduce waiting time to see a physician to less than 15 minutes within 9 months.” (IHI.org). In this example, you would track waiting times over a period of time.
