Community Resources for Students with Disabilities
One aspect of the transition process is connecting students with disabilities and their families with community resources and agencies that will support them once they leave the public school setting. In order to do this, special educators must be aware of services that are available in their communities. Using the SED 502_Session 4 Community Resources Template.docx, put together a document of community resources that can be provided to students with disabilities and their families. If you live/work in a rural community that has minimal resources, your document may include state-wide or national resources to support adults with disabilities. As you put together this document, consider the following categories of resources that may be available:
Workforce support/Job coaching
Disability services at community colleges and universities
Health services
Recreational services
Case management
