To help you identify such a practice problem, think about when you have asked yourself, “Why are we doing this?” or “Why are we doing it this way?” Practices to consider are those that have (1) high volume, (2) important clinical outcomes, (3) significant adverse effects, (4) reporting requirements for quality improvement or regulatory agencies, and/or (5) serious financial implications.
