Include a Routine: Arrival Times, Assigned tasks, Involvement in implementing Activities, Participation in assisting clients/ service users with activities of daily living, provide/draw a clear timetable in each diary for each day (there should be some variation from the standard routine plan on a day to day basis.) 3. Discuss assigned tasks carried out e.g. assisting client/service users with personal care, toileting, mobility, feeding 4. Highlight/Discuss Communication with all Stakeholders – Clients/Service users, familiy and Staff. 5. Identify Challenges/Rewarding situations which may have arisen – issues and challenges encountered by you during your Work Experience Hours. Choose 7 days but again choose 7 busy, varied and interesting days. Work Experience Diary – construct each diary entry using the headings listed below for each working day using the additional Information for guidance. Include information on the following: The client/service user profile you were working with on each specified day i.e. age range, conditions, dependence level, gender etc. Number of clients/service users in your care on specified day. Staff ratio levels/skill mix on the specified day. Routine of the Care Setting Highlight a clear routine of each day followed by the learners Healthcare Setting. Present a clear timetable format then provide details of the Daily Routine followed including times of handovers( every morning at 8.00 am meeting of handover) , assistances with ADLs, lunch/tea times( tea time at 11.00 am ) , activities etc. Daily Living Tasks Discuss involvement or experience gained by completing Daily living tasks i.e. assisting with feeding, toileting, personal hygiene needs, mobility, recreational and rehabilitation activities, pressure area care, catheter care, specific clinical skills. Communication with All Stake Holders: Interaction with Clients / family / Staff Describe interactions with clients/service users, family members and staff, which may have occurred during your daily routine (please appreciate that not all interactions occur every day) Did you enjoy working with others? What interaction had you with family members? Were you comfortable talking to family members? Were the Staff helpful? Did you help them? Did you ask any questions? Did they mind answering? Did you follow instructions? Did they give you any feedback? Identification of Situations / Issues which were Rewarding or Challenging during your Work Experience? Identify situations which may have arisen during your Work Placement both rewarding or challenging? Diary that need to be written: 1. Assisting a client with feeding who had swallowing difficulties 2.Dealing with challenging behaviours 3.Communicating with a client with sensory difficulties 4. Bathing a client who have mobility problems 5. Dressing/undressing a client with dementia 6.Transferring a client from bed to wheelchair
