A 69-year-old woman with a long history of hypertension (HTN), coronary artery disease, arthritis, asthma, obstructive sleep apnea, hyperlipidemia, and diabetes mellitus was diagnosed with resistant HTN 3 years ago. Her BP was always uncontrolled (>140/90 mm Hg. At her first cardiology appointment, her BP was 160/90 mm Hg when she was taking amlodipinebenazepril (520 mg daily), verapamil (240 mg daily), and valsartanhydrochlorothiazide (320/12.5 mg daily).
Her antihypertensive medication was then adjusted to spironolactone (50 mg daily), valsartanhydrochlorothiazide (320/25 mg daily), and verapamil (360 mg daily), and her BP was still 150/80 mm Hg.
The consulting cardiologist understands that there has been ongoing debate about the impact of dietary salt restriction on BP control. She turns to the recent meta-analysis and systematic review literature for some guidance on the value of initiating a salt restricted diet for this patient who is already living with dietary constraints.
A 2020 paper in the renowned BMJ was selected since it utilized Ovid MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials that collectively analyzed data from 133 studies with over 12,000 subjects, all focused on the effect of reduction of dietary sodium on BP.
After all manner of data analytics and elegant statistics applied across all groups, ages, gender, and health status, it was concluded that dietary sodium reduction resulted in statistically significant decrements in systolic and diastolic pressure, and that “potentially important health benefits would accrue to normotensive as well as hypertensive patients.”
The attending cardiologist decides to recommend strict compliance with a marked restriction of dietary sodium.
