Develop a 10 question customer satisfaction survey including the questions and gradient scale.
Hypothetically a 5% response rate was achieved and an overall score of 65% for the survey results. What opportunities for improvement did you identify and what plan for follow-up would you recommend? Be as specific as possible and be creative.
