CASE 18: The Shy Patient
Cheryl Cameron
University of Washington
A new pediatric patient presented for a dental prophylaxis with the dental hygienist. She was an 8-year-old, shy, and reserved child who was cute and well mannered. The dental hygienist took an immediate liking to this patient. She had generally good personal and oral health. However, her maxillary central incisors were fractured. When asked about how these teeth were fractured she reported having fallen off her bike. Unfortunately, she was not wearing a helmet.
At her recall appointment 6 months later, the dental hygienist noticed that although her incisors were restored, she had bruising around both eyes. When asked about the bruising she reported having fallen down the stairs at home. Her mother was present in the room and holding her hand but offered no further explanation. A review of the patient record revealed that this patient sought care by three different general dentists in the same city before transferring her care to the current office.
Questions
1. Should the dental hygienist suspect child abuse?
2. What professional obligations does the dental hygienist have to the patient?
3. How should the dental hygienist document his/her observations?
4. How should the dental hygienist proceed?
